Off the wagon? When Matthew Perry was spotted out on a dinner date in West Hollywood on November 4, he wasn’t looking too hot. Now, a source reveals the situation may have been even messier than it seemed. An eyewitness account exclusively shared with In Touch that the Friends star seemed to be struggling.

“Matthew was disheveled, he seemed really out of it,” the insider told In Touch. “He was laughing, swerving and swaying.” Revealing that the girl he was with was “comforting him,” the source said, “She felt sorry for him. It didn’t seem like a girlfriend situation at all. She was definitely comforting him, rubbing his arm, trying to talk to him about getting home, getting some rest. He was very messy, sloppy. He looked like a homeless man.”

The whole thing was caught on camera as the mystery woman helped him to his car. “The paparazzi was out here, watching his every move,” the insider added. “The girl wasn’t mad at him, but she was definitely treating him kindly, like she knew he was a mess and wanted to make sure he got home OK. He was smoking; he [didn’t seem] sober.” The good news? “He did not drive. The girl got him a car, I think it was an Uber.”

“Matthew was feeling no pain, [but] he didn’t look good at all, definitely didn’t seem to care about his appearance or anyone seeing him,” they continued. “It was kind of sad.”

A second source confirmed to In Touch that the star had “dinner and drinks” and sat at a booth inside the restaurant. After first heading to rehab in 1997 and again in 2001, the Friends actor has struggled with his sobriety, relapsing more than once over the years. In 2011, he headed off to rehab again. “I couldn’t stop. Eventually, things got so bad that I couldn’t hide it, and everybody knew,” he admitted in 2013. “You can’t have a drug problem for 30 years and then expect to have it be solved in 28 days. Getting sober is really a hard thing to do.”
